All the names of patients and family members have been changed to protect their privacy. Some identifying details have been changed.
Most of the names of physicians have been changed.
“Chaim K.” is a nom de plume of the author's co-writer in the essays in the section titled “Life-Wizened Chaim K.”
While the articles and essays are divided into sections according to a general topic, there is some overlap of topics. Some essays can be placed in two or even three sections. For example: “When the Deer Hunter Lost” is suitable for the sections on Dates of Sweetness and Awe, Israel in Colorful Times, and Living Torah where it was finally placed.
